Job Description

Provides high level of support for developing and implementing internal and external initiatives intent on improving the care coordination/care transition processes bi-directional between acute and post-acute care settings.
Provides high level of support for community based initiatives focused on health improvement with the intent of improving the overall health of the community.
Must be able to apply principals of logic and data processing concepts to define problems, collect data, establish facts, and draw valid conclusions.
Must understand current environment of healthcare and be knowledgeable of new trends in clinical practices and information systems.
Must be able to apply current analysis and reporting techniques.
Ability to coordinate project management functions to include identification of successes, barriers, and opportunities. Must maintain project management documentation.

Collaborate in the development of and contribute to individual, team, and organizational improvement activities.
Coordinate and participate in community activities that promote health and prevent disease for various population groups. Settings include but are not limited to: Acute Hospital, post-acute facilities, physician groups, community organizations, businesses, schools, and the general community.
Demonstrate ability to see a project or assignment to completion, accepting responsibility for all aspects of the project.
Maintain master files and/or reports as necessary to support end users and the goals of the system.
Maintain schedules, payroll and time corrections. Enter departmental charges and maintain batch data.
Assist with the collection of data for various dashboards, project documents, process improvement processes, and daily operations.
Track, analyze, and evaluate data that impacts quality outcomes and strategic initiatives.
Develop and prepare flow charts, graphs, and other data trending as directed.
Facilitate CNE/CME education sessions as identified by needs assessment activities.
Manage workload prioritization and progress for members of the leadership team.
Update and distribute recall roster for each quarterly review.
Coordinates department meetings; records, types and files minutes. Files all correspondence, reports, and forms in an appropriate location and in an orderly fashion within specified time frame of department.
Handles correspondence as required and/or requested by the director. Maintains department records and established department filing system to ensure the expeditious retrieval of.
Assists in contract development, maintenance, and filing.
Performs all other tasks/responsibilities as necessary.

Working hours: 8:00 AM - 5:00 PM

Experience with computers required.
Ability to type sixty (60) wpm for computer data entry ensuring correct spelling and grammar.
Primarily works the day shift tour of duty; varied hours may be necessary on occasion.
Requires prolonged standing and sitting, along with frequent bending, stooping, stretching.
May require lifting up to 25 pounds.
Requires eye-hand coordination and manual dexterity.
Corrected vision and hearing to normal or near normal range.


4-7 years

Degree from University preferred.
Understanding of medical terminology.
Five years??? experience in the acute care hospital or community-based healthcare environment

Must submit to drug screening and background check.
