Job Description

This role makes an impact in long term care community programs assisting with eligibility determinations and reviewing medical information to provide expert advice on support needs.

This position is responsible for determining Medicaid eligibility and consultation on a statewide basis for children with long-term support or complex medical needs, permitting those individuals to receive highly complex health care services in their homes/community in lieu of extended hospital/institutional placement.

-Coordinate the gathering of confidential and highly complex medical information on a case-by-case basis for all applicants and current participants in the Katie Beckett and Children's Long-Term Support Programs.
-Review applications, functional eligibility determination and annual reviews of approved cases to assure continued compliance with federal guidelines.
-Provide individual pediatric nursing consultation to parents and providers of health care and social services for all children with developmental or long-term support needs. Review, analyze and provide consultation on nursing and support services as needed.
-Provide expert, statewide pediatric nurse consultation to other department staff, county agencies, hospitals, long-term care facilities, families and other providers relating to the assessment, planning, implementation and evaluation of appropriate services for children in their homes and communities.

Working hours: 1st shift Monday-Friday

-Bachelor's degree in Nursing or similar field with extensive medical knowledge preferably pediatric nursing
-Extensive knowledge of the implications of long-term support needs as they relate to service and support costs and appropriate provision of community-based services.
-Extensive knowledge of individual assessment, planning, intervention and evaluation techniques.
-Considerable knowledge of the implications of medical conditions as they relate to medical services costs and appropriate provision of community-based services.
-Considerable knowledge of the implications of pediatric mental health problems as they relate to medical services costs and appropriate provision of community-based services.
-Knowledge about the development and analysis of public policy related to long-term support.
