Job Description

The RN Case Manager is responsible for the planning and delivery of patient care, providing skilled nursing care and oversight in accordance with policies and the Florida Nurse Practice Act, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. To provide a combination of skilled professional nursing tasks and provide palliative and supportive care to the patient and family unit.

- Provide program development strategies for client, in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Inspire staff to live the client's Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.
- To meet or exceed identifiable standards for the position.
- Must have the capacity to embrace a flexible work schedule to meet the patient and family needs.
- Participates as an active supporter of the client's compliance program, including the emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization's compliance program.
- Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.

Working hours: 8:00 AM - 6:00 PM

- Incumbent may be required to perform emergency duty before, during and/or beyond normal work hours or days in the event of an emergency, crisis situation or disaster (man-made or natural) including evacuation sites.

1-4 years

- Current and active Florida RN license.
- Must be Certified Hospice and Palliative Registered Nurse (CHPRN) within 12 months.
- Current and valid Florida driver?s license and automobile insurance.
- Current CPR certification required.
- Minimum of two years RN Pediatric Nursing experience, as applicable.
- Minimum of two years RN experience where physical assessment skills were utilized.
- Prior experience in oncology, home health, Hospice or palliative care setting is preferred.
- Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.
- Must be able to effectively plan, organize and prioritize work.
- Must be self-directed and able to work independently in the field.
- Basic computer skills and EMR software experience preferred.
