You will leverage your existing network of local agencies, government resources, and community organizations to connect Veterans with vital services such as housing, healthcare, employment, and mental health support. Your ability to build trust and foster relationships will be essential in empowering Veterans to achieve stability and improve their quality of life.
Responsibilities:
Conduct thorough assessments of homeless veterans to identify their individual needs, challenges, and goals.
Develop and implement personalized case plans, incorporating short-term and long-term goals for housing, employment, healthcare, and overall well-being.
Provide crisis intervention, counseling, and advocacy to address immediate needs and facilitate access to appropriate resources.
Collaborate with community partners, government agencies, and other service providers to ensure a coordinated and effective network of support for veterans.
Assist Veterans in navigating social services, housing programs, and healthcare systems to ensure they receive the necessary benefits and support.
Maintain accurate and up-to-date case records, progress notes, and documentation in compliance with organizational and regulatory standards.
Conduct regular follow-up meetings with veterans to monitor progress, address challenges, and adjust case plans as needed.
Advocate for Veterans' rights and entitlements, ensuring they receive fair and equitable treatment in all aspects of their lives.
Working hours: 8:00 AM - 5:00 PM

Bachelors
Experience:
1-4 years
Qualifications:
BA in social work, psychology or related field, or equivalent combination of education and experience
Minimum 1 year experience in homeless prevention or housing navigation
Ability to work independently and as part of a team, with minimal supervision.
Ability to communicate effectively, both verbally and in writing, with diverse populations.
Ability to handle multiple tasks and prioritize workload, in a fast-paced environment.
Ability to use computer and software applications, such as Microsoft Office, HMIS, etc.
