-Develop change management plans to roll out programs including delivery of communications and learning-Assist CARES Culture Committee with tracking of key metrics and completion of deliverables-Create daily controls to manage production and delivery of communication (e.g., email, video) and implementation plan deliverables (e.g., training interventions, patient education, website changes)-Collaborate with CIT and onboarding teams to ensure communication methods are effectively optimized and integrated within onboarding plan-Collaborate with Learning and Development, CIT, Marketing and other key collaborators to create appropriate change interventions (e.g., training, video, workflow design, reports)-Design & continuously refine service model process, roles, tools, and metrics-Conduct pilots to ascertain best practices for future implementation into the value-based care model by leveraging interview sills to gain patient, team member, partner, and service delivery insights/feedback to improve service capabilities and test new concepts that will inform transformation initiatives and continued iteration on those improvements-Partner with other teams within the organization to ensure that each team has an appropriate consistency across conventional and value-based service delivery
