Responsibilities:
1. Provide care and supervision to residents.
2. Assist with activities of daily living, including medication oversight, follow facility
protocol, licensing regulation and guidelines.
3. Follow the individual service plan and for each client.
4. Function as a team, assisting coworkers as the need arises.
5. Monitor client activity, food intake, functional status, psychosocial status, taking action
as required to promote client well being.
6. Report status change immediately to the supervisor.
7. Document client status change, including but not limited to, physical change, reaction to
medication, psychosocial status change, and/or behavioral changes.
8. Document client rehabilitation services provided.
9. Adhere to guidelines in the employee handbook including dress code, code of conduct,
scheduling, etc.
10. Other duties as assigned.
Working hours: Various Shifts Available
Skills:
Basic First Aid
CPR certification is required.
Education:
High School
Experience:
1-4 years
Qualifications:
High School Diploma or equivalent.
Basic First Aid CPR certification is required.

Additionally, the individual must possess the capability to read, understand and carry out client service plans and document activities.
