Our client, local non-profit health facility, is in immediate need of a professional to provide transportation with a company van to and from a variety of places for clients with mental illness. This includes transporting to and from medical appointments, assisting them with the process as needed and taking part in planned outings and day program activities.
Responsibilities:
Candidate will will provide supervision for clients while in his/her care. Promoting client well-being and satisfaction through support with activities of daily living is an important part of the position. Medical appointments and grocery shopping for four residential facilities is also part of the position.
Responsibilities:
1. Provide and coordinate transportation and client support at medical appointments and when in the community.
2. Provides logs of daily activities and Medi-Cal billing documentation. If no appointment or trips are scheduled, report to Administrator or Director of Residential Services for directives.
3. Grocery shopping for 4 Adult Residential Facility houses. Handle errands and other assignments that require driving.
4. Work during shifts as needed and when time permits.
5. Function as a team, assisting coworkers as the need arises.
6. Other duties as assigned.
Working hours: 7:00 AM - 5:00 PM

Qualifications:
Must be able to communicate effectively with the clients, staff and supervisors. Additionally, the individual must possess the capability to read, understand and carry out service orders for clients.
