We are looking for a dedicated and compassionate Registered Nurse (RN) Case Manager to join our Client team. As an RN Case Manager, you will play a crucial role in coordinating comprehensive home care for our Geriatric clients, creating custom care plans, and supervising Caregivers. Our Client offers the opportunity to work closely with patients, families, and caregivers to ensure the highest quality of care in the comfort of their homes. the RN Manager has full autonomy over thier schedule, the ideal week will be broken down into the following 1 day client assessments and visite, 2 days in the office, 2 days working from home.

Responsibilities:
Conduct initial client assessments and develop individualized care plans.
Assign proper care giver to the patient based on their needs.
Provide health education, and emotional support to clients and their families.
Monitor client progress and update care plans as needed.
Monitor Caregiver's performance and evaluate at 90 days and annually.
Educate clients and caregivers on disease processes, treatment options, and self-care techniques.
Advocate for client?s needs and collaborate with healthcare providers to ensure continuity of care.
Document client care and maintain accurate medical records in compliance with agency policies and regulatory requirements.
Participate in case conferences, team meetings, and in-service education sessions as required.
Be available for on-call work in emergency situations - infrequent.
Working hours: 8:00 AM - 5:00 PM
Skills:
Strong clinical assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently and as part of a multidisciplinary team.
Reliable transportation and willingness to travel within the metro Atlanta Area.
BLS certification.
Proficiency in electronic health records (EHR) systems.
Education:
Bachelors
Experience:
4-7 years
Qualifications:
Current RN license in GA (required).
Nursing Degree, BSN preferred.
Minimum 5 years of clinical nursing experience (required).
Minimum of 2 years of Clinical Manager experience, preferably in home care or Home Health care or a related setting (required).
