Job Description:

We are seeking a compassionate and detail-oriented Licensed Practical Nurse (LPN) to join a care team in Clearfield, PA. As an LPN you will work closely with residents, families, and interdisciplinary staff to ensure the health and well-being of our residents. You’ll play a key role in monitoring resident health, administering medication, and supporting daily living activities in accordance with state regulations and facility policies.

Responsibilities:

• Deliver direct nursing care to residents
• Administer medications and treatments as prescribed
• Monitor and document resident conditions and report changes to appropriate personnel
• Provide leadership to caregivers and support staff
• Communicate effectively with residents, families, and medical professionals
• Ensure compliance with resident care plans and facility protocols
• Promote a safe, clean, and comfortable living environment

Education: Professional Certificate

• High School Diploma or GED
