Are you passionate about enhancing services for older adults and people with disabilities? The Wisconsin Department of Health Services (DHS) is seeking a dedicated Adult Protective Services (APS) No Wrong Door (NWD) Coordinator for an 18-month contract position. This role will support a federal grant to expand and improve Wisconsins NWD system, which helps people access community services easily and efficiently.
This position is vital in ensuring older adults and adults with disabilities who are currently served by APS can access the long-term services and supports they need to maintain their independence. Youll work with another NWD grant coordinator to support grant management and reporting requirements.
Responsibilities:
- Needs Assessment: Complete an APS system needs assessment to identify gaps, strengths, and barriers in services and referral processes between APS and ADRC systems.
- Outreach and Coordination: Develop and share educational materials and tools for APS and ADRC staff. Create resources for adults in crisis and help with the development of NWD consumer and professional materials.
- Training and Technical Assistance: Develop and host trainings for APS, ADRC, and Tribal program staff to enhance NWD system awareness and collaboration.
- System Partnerships: Coordinate with partners serving adults at risk and those in crisis to promote an enhanced NWD approach.
- Reporting: Develop project summary reports and present outcomes to leadership and NWD partners.
Qualifications:
- Experience: Direct experience working in county APS, ADRCs, or Aging Services programs.
- Attributes: Ability to work both independently and as part of a remote team.
- Knowledge: Familiarity with the Wisconsin human service delivery system for older adults and people with disabilities.
- Strong project management and organizational skills with exceptional attention to detail.
- Excellent analytical skills to collect, organize, and disseminate information.
- Exceptional oral and written communication and interpersonal skills to provide guidance and foster strong relationships.
- Proficiency in Microsoft Word, Excel, PowerPoint, Teams, and Zoom.
- Experience with grant management procedures, including data collection and reporting.
- Knowledge of Medicaid Long-term Care eligibility and policy.
- Experience with the core principles of "No Wrong Door" systems, including person-centered practice.
- Experience working with adults from culturally diverse backgrounds, including Tribal members.
- Experience developing educational resources or training materials.
- Familiarity with trauma-informed and culturally sensitive services for adults in Wisconsin.
